HS Health Notices
Greetings from the High School Health Office!
Here is some helpful information for parents/guardians of Cazenovia High School Students.
Please reach out with any questions or concerns you may have.
Health Appraisals:
A physical examination including Body Mass Index and Weight Status Category Information is required for all newly entering students and students in grades 9 and 11. The form must be submitted no more than 30 days after the start of school. While not required for school attendance, a dental certificate is also requested at this time.
Please Note: Student athletes require a yearly physical exam, and must register for sports in FamilyID.
Medications in school:
If a student will need prescription or over the counter medication administered at school; the “medications in school” form must be filled out by both the physician prescribing the medication and the parent. These forms must be updated every school year. If your child will self carry medication, this must be indicated on the form. Forms can be found under the medical forms tab on the main page.
Immunizations:
All students entering grade 12 are required to receive a second Meningococcal A vaccine given after their 16th birthday by NYS Public Health Law. This vaccine must be administered and record given to the school nurse no later than 14 days after the start of school.
Absences/Sickness Dismissal:
If your child is absent, please notify me or Mrs. Haube in the main office in writing with the reason for the absence. Students are not permitted to text/call parents for a sickness dismissal. All sickness dismissals must go through the health office. Your cooperation is appreciated.
