Hello from the Middle School Health Office!
Here is some helpful information for parents/guardians of Cazenovia Middle School Students:
Health Appraisals (physicals) are required for all new students entering the district and all students entering 5th and 7th grades. They must be submitted to the nurse’s office no more than 30 days after the start of school. A health appraisal is valid for 12 months. A dental certificate is requested at this time as well.
Modified Sports- Students may start playing modified sports in 7th grade. Student athletes are required to have a valid physical and register for the sport on FamilyID.
6th Grade Tdap- All students entering 6th grade are required to receive a Tdap vaccine, according to NYS Public Health Law. The vaccine must be administered and record given to the school nurse no later than 14 days after the start of school. If your child turns 11 after the start of school, NYS allows a grace period of 14 days after their 11th birthday to receive the vaccine.
7th Grade Meningococcal Vaccine- All students entering 7th grade are required by NYS Public Health Law to receive a meningococcal vaccine. Students must receive it no more than 14 days after the start of school.
Medications- Students who require medications in school must provide the school with a doctor’s order (for the med, dose, time and frequency) each school year. Medications must be in the original container/package. If your child will be self carrying the medication, it must be indicated on the doctor’s order.
Absences/Sickness dismissal- If your child is absent, please notify me or the main office in writing, (either email or by hand) for the reason your child was absent. Students are not permitted to text/call parents for a sickness dismissal. All sickness dismissals must go through the nurse’s office.
